To ensure both the client and therapist's boundaries are clearly defined and kept safe, it is imperative that professional standards are maintained. Thank you for reading and honoring my policies.

• Clients are expected to be clean.
• Clients may not be intoxicated in any way.
• Clients must reschedule if they are sick, infectious, or under the influence of drugs or alcohol.

Appointments & Arrival
• First-time and returning clients should arrive no more than 5 minutes before their scheduled session.
• If a client is late, the session may be shortened to avoid disrupting the schedule.
• If significantly late, the therapist may choose to reschedule, and a 75% of Appt. no-show fee may apply.

Cancellations & No-Shows
• Cancellations made at least 24 hours in advance will not be charged.
• Cancellations within 24 hours will be charged $25 to the card on file OR must be paid via invoice before rescheduling.
• No-call, no-shows will be charged 75% of the scheduled service to the card on file.
*Please CALL and leave a voicemail if you need to cancel your appointment. Please DO NOT cancel through Facebook Messenger, contact forms, or text message because I do not always see these in time.*

Right to Refuse Service
MK Massage Therapy & Wellness Co. reserves the right to terminate any session or refuse service at the therapist’s discretion for reasons including inappropriate behavior, illness, intoxication, poor hygiene, or repeated policy violations. If a session is terminated, the therapist will determine whether the client is charged and whether they may book future appointments.

Non-Sexual, Professional Service-
MK Massage Therapy & Wellness Co. provides professional, non-sexual massage therapy. Any inappropriate behavior, conversation, or gestures toward the therapist will be considered sexual harassment. If this occurs, the session will be immediately terminated with no refund, payment will be required in full, and the client will no longer be allowed to receive services.

Health Forms & Consent
• Clients must complete a brief health history form and sign the office policy agreement before receiving treatment (completed above).
• Clients under 18 must provide written consent from a legal guardian.

Confidentiality & Ethics
• All personal information and conversations between client and therapist are confidential and protected under HIPAA standards.
• Your massage therapist follows the NCBTMB Code of Ethics.

Privacy & Comfort
• Your privacy will always be respected. The treatment room is private. Appropriate, professional draping will be used at all times to ensure only the area being worked on is exposed.
• Clients may undress to their comfort level. At no point will inappropriate or unnecessary exposure occur.
• Please feel free to ask any questions or express any concerns.

Medical Disclaimer
• Any advice given is for general wellness purposes only and should not be considered a medical diagnosis.
• Massage therapy is not a substitute for medical care. Your therapist cannot perform spinal or skeletal adjustments, diagnose conditions, prescribe medication, or treat illnesses.
• If you experience pain or discomfort during your session, inform your therapist immediately so adjustments can be made.
• Your therapist is not liable for any pain or discomfort experienced during or after the session.

Payment Policy
• A valid credit or debit card is REQUIRED to book an appointment.
• You may choose to pay at the time of service with cash or card if you select the “Reserve without Paying” option during scheduling.
• Your card will not be charged unless you cancel within 24 hours or no-show.

Client Agreement
By signing this release at the time of booking your appointment, you affirm that:
• You have disclosed all known medical conditions and injuries.
• You will inform your therapist of any health changes before each session.
• You release your therapist from any liability, past, present, or future, related to massage therapy and bodywork.
• You agree to all the terms above.
